By registering with Polo by the Pier (PBTP), you (and, if you are under 18 years of age, you and your parent or legal guardian whose name and details appears in the requisite fields):
1) Hereby are affiliated to Polo by the Pier (PBTP) as an individual competitor.
2) Agree that the information provided by you is current and correct at time of registration and will be updated as required by yourself.
3) Acknowledge that water polo is an inherently risky sport and that you accordingly accept all risks involved in competing and otherwise participating in activities related to or forming part of your affiliation to Polo by the Pier and your registration to participate in the event.
MOUTHGUARD POLICY
1) I acknowledge that PBTP PBTP has adopted a policy which makes the wearing of mouthguards compulsory during matches conducted or authorised by PBTP PBTP.
2) I acknowledge that I have freely made the a decision above whether or not to I wear a mouthguard while competing/engaging in any activity conducted by Polo by the Pier;
3) I release and forever discharge PBTPPolo by the Pier, (PBTP) and any of their agents or employees and any other person from and against all claims, actions, suits and demands of every description whatsoever which, but for the execution of this Agreement, I could or might have, by reason of, or arising out of my decision not to wear a mouthguard during water polo matches.
DECLARATION BY APPLICANT
1) I hereby agree to accept and abide by the respective Rules, Regulations, policies and guidelines of my Club and PBTP including Safe Sport Framework (goo.gl/Ut1YYH) and Member Protection policies and that failure to comply may result in disciplinary action.
2) I further grant permission for any photographic and/or video recording of me taken during any event conducted by PBTP Polo by the Pier (PBTP) or my registered club to be displayed, published or otherwise used for the purpose of advertisement and/or promotion of the event.
3) To the best of my knowledge, the information given in this application form is true and correct and I acknowledge intentionally invalid or misleading information may result in my registration being SUSPENDED and therefore be INELIGIBLE for competition.
4) I tender the appropriate fee necessary for the consideration and acceptance of my application for registration.
5) I acknowledge that competitive sport involves real risk of serious injury or even death from various causes including; dehydration, physical stress and health failure; equipment failure; accidents with other competitors, spectators or road users; drowning, inherent race course risks; weather conditions and other causes. I acknowledge that the leading cause (more than 90%) of non-traumatic sudden death of athletes is related to a pre-existing cardiac abnormality.
6) By competing, I accept all risks necessarily flowing from my participation which could result in loss of life or permanent injury. Accordingly, I release all persons or corporations associated directly or indirectly with the conduct of the event from all claims demands and proceedings arising out of my participation and I hereby indemnify them against all liability (including liability for their negligence and the negligence of others) for all injury, loss of damage arising out of or connected with my participation in this event. This release shall extend to and include PBTP Polo by the Pier (PBTP) and their respective directors, partners, managers, officers, agents, contractors, employees and volunteers including medical and paramedical personnel appointed for the event, the owners, licensees and occupiers of land upon which the event or any part of it is conducted, any statutory body or local authority having control over any land upon which the event or any part of the event is conducted or which is involved directly or indirectly with the event in any manner whatsoever and promoters, sponsors and event organisers. This release and indemnity continues forever and binds my heirs, executors, personal representatives and assigns.
7) I am fully responsible for the security of my personal possessions at the event including my bike and other valuable items during the event and across the entire operational area. PBTP Polo by the Pier (PBTP) will not be liable for replacement of any personal possessions at any time or in any location.
FOR PARENTS/GUARDIANS OF PARTICIPANTS UNDER AGE 18
This is to certify that I as parent/guardian with legal responsibility for this participant, do consent and agree to the disclaimer and release of liability as provided above, and, for myself and my heirs, assigns, personal and legal representatives and next of kin. I hereby release and agree to indemnify and hold harmless the Releasees from any and all claims and liabilities incident to my minor child’s/ward’s involvement or participation in this event as provided above, to the fullest extent permitted by law.
